Perspectives of new imaging techniques for patients with known or suspected coronary artery disease

چکیده
Coronary artery disease (CAD) remains one of the leading causes of morbidity and mortality in western society. Over recent decades, major advances have been made in non invasive imaging techniques, resulting in an enormous growth in their role in the clinical assessment of patients with known and suspected CAD. Indeed, although invasive coronary angiography remains the current gold standard in the diagnosis of CAD, first-line non invasive assessment with functional techniques has become essential, particularly in patients at intermediate risk for CAD, to select those patients needing further invasive evaluation and intervention. For this purpose, nuclear imaging has since long been available, while more recently functional imaging can also be performed with stress-echocardiography (with contrast) and magnetic resonance imaging (MRI). Importantly, these techniques visualize the consequences of obstructive CAD, namely ischemia, rather than the coronary arteries directly, and the presence of atherosclerosis that is not flow limiting cannot be evaluated. For the latter purpose, non invasive coronary angiography with MRI and computed tomography techniques is under extensive investigation. The purpose of this review is to provide an overview of the non invasive imaging techniques currently available in the evaluation of CAD. Heart Metab. 2007;34:10–14.

THALLIUM-201 STRESS TEST IN 156 PATIENTS WITH SUSPECTED CORONARY ARTERY DISEASE
Currently, myocardial TI-201 scintigraphy is most often performed in conjunction with exercise stress testing in patients with suspected or known CAD. Stress thallium tests were performed for 156 patients with and without old myocardial infarctions complaining of chest pain. All of them underwent contrast coronary angiography. Perfusion defects were mostly manifested as reversible defects ...
متن کاملThe prognostic value of stress/rest gated myocardial perfusion SPECT in patients with known or suspected coronary artery disease
Introduction:Gated myocardial perfusion SPECT has diagnostic and prognostic values in coronary artery disease (CAD). We tried to determine prognostic values of the left ventricular perfusion & functional indices as well as transient left ventricular dilation (TID) derived from gated myocardial perfusion SPECT. Methods: 1820patients who underwent gated myo...
متن کاملCoronary artery calcium (CAC) score – a prognostic tool in coronary artery disease?
Introduction: The aim of this study was to evaluate the impact of measurement of coronary artery calcification score (CAC) in patients with suspected coronary artery disease (CAD) and a normal myocardial perfusion scan. Methods: In a prospective study we measured the calcium score of 74 patients (29 m, 45 f, mean age 58.7 (m) and 64.4 (f)) with suspicion of CAD and a normal perfusion scan. In ...
متن کاملPerformance of myocardial perfusion imaging using multi-focus fan beam collimator with resolution recovery reconstruction in a comparison with conventional SPECT
Objective: IQSPECT is an advanced high-speed SPECT modality for performing myocardial perfusion imaging (MPI), which uses a multi-focus fan beam collimator with resolution recovery reconstruction. The aim of this study was to compare IQSPECT compared with conventional SPECT interms of performance based on standard clinical protocols. In addition, we examined the concordance between convention...
